About LICIL:

The Long Island Center for Independent Living, Inc. (LICIL) is a not-for-profit 501(c)(3) organization advocating for and serving the needs of people with disabilities since 1980. Non-residential and consumer controlled, LICIL is dedicated to the advancement of public policy which ensures and prioritizes equity, access, and opportunity for all persons with disabilities. Experienced staff provide advocacy and direct services to individuals and families across the lifespan and across all disability experiences.  Direct services include: advocacy, benefits advisement, housing guidance, peer counseling, Independent Living skills training, equipment loan, and ADA consultation.  Our Consumer Directed Personal Assistance (CDPA) service is available for Medicaid eligible individuals who wish to self-direct their home care.


Customized Employment Specialist

About the Position:

This position is part of the Subminimum Wage to Competitive Integrated Employment
(SWTCIE) Program. The individual in this position will increase opportunities for individuals with developmental and or other significant disabilities who are contemplating employment, are employed, or were employed at subminimum wage to assist them with the discovery and delivery of educational, vocational, and employment support programs and resources to obtain competitive integrated employment to help them achieve their desired level of independence.

Education and Training:

Minimum of a Bachelor’s degree or an equivalent combination of education and experience to successfully perform the essential duties of the position may be substituted.

Specific Duties/Skills:

  • Assist consumers in the development of an Independent Living Plan (ILP).
  • Document the development and achievement of IL (Independent Living) goals selected by individuals.
  • Ability to keep an open mind in learning, practicing and fully adopting the more intensive Customized Employment (CE) service process, including the length of time and performance based mentoring components involved.
  • Flexibility in scheduling to accommodate the customized employment process; especially employment support which may entail some after hours/weekend work.
  • Maintain consistent contact with consumer and employer and plan regularly scheduled meetings.
  • Prepare and maintain employment records, coach employers, renegotiate salary, employment evaluations, promotions, provide benefits advisement, and all aspects of employment support.
  • Commitment to develop and maintain vocational/employment opportunities for individuals with disabilities (including individuals with the most significant impact of disability and/or limited to no work history) matching the customer’s unique strengths, abilities, needs, interests, and abilities.
  • Collaborate with people with disabilities, the CE Specialist, family members, direct support professionals, VR (Vocational Rehabilitation) staff, extended service providers, and employers
  • Meet with consumers in their homes and in the community to provide ongoing support services, set up and training of extended services to ensure long-term employment.
  • Conduct and document activities for initial phases and employment phases after on-boarding customized employment.
  • Follow the prescribed policies and procedures when implementing (CE) services.
  • Participate in (CE) systems analysis, ongoing program evaluation, continuous quality improvement data collection and the provision of information.
  • Participate in (CE) mentoring sessions by email, phone, Teams, Zoom, etc. and with their assigned technical assistant for each phase and stage of the (CE) process.
  • Register and complete assignments and tasks in the (CE) Mentoring Web Based Platform.
  • Record the time it takes to complete identified (CE) task related to training.
  • Participate in Customized Employment Community of Practice
  • Deliver (CE) Services that meet best practices.
  • Obtain and maintains the (CE) Association of Community Rehabilitation Educators Certificate (ACRE)
  • Accurately documents consumer services and required paperwork in LICIL’s Electronic Consumer Record (ECR) within two (2) working days of the service
  • Attend agency’s in-service training and staff meetings as well as any other agency-related activities as instructed.
  • Participate in maintaining a neat, clean, and safe work environment.
  • Perform other job-related duties as assigned by the supervisor.

Experience:

Housing Specialist

 About the Position:

The Housing Specialist will report to the Executive Director and will work collaboratively with the Rapid Transition Housing Program (RHTP) Independent Living (IL) Specialist, as well as other programs at LICIL. The Housing Specialist’s key role is to Identify independent housing opportunities for unstably housed individuals who are either in an inpatient setting or unstably housed in the community and to assist eligible participants through all steps necessary for enrollment and admission.

Specific Duties/Skills:

  • Identify independent housing opportunities for unstably housed individuals who are either in an inpatient setting or unstably housed in the community.
  • Assist eligible participants through all steps necessary for enrollment and admission, including: acquiring eligibility documentation, filling out housing applications, obtaining household goods and/or support for moving expenses, completing unit inspections, etc.
  • Collaborate with applicants and/or their Independent Living Specialist, Transition Specialist, Service Coordinator (SC), Nursing Home Discharge Planner, Care Manager (CM), natural support, or advocate ensuring a support services plan is in place for program enrollment.
  • Conduct follow-up after subsidy is initiated including but not limited to verifying occupancy of unit, continued eligibility for program, and assist in completing energy assistance applications.
  • Work to identify landlords, public housing, and private housing in the region which may be suitable for individuals enrolled in the program in the future.
  • Maintain a list of available housing in the region, including degree of accessibility, location, proximity to services, and rental costs.
  • Attend trainings and perform other duties as requested by the Program Director or the Chief Executive Officer.
  • Track and report on all activities using the statewide project database.

Qualifications:

  • Minimum of a Bachelor’s degree or an equivalent combination of education and experience to successfully perform the essential duties of the position
  • Professional experience in housing, community organizing, Independent Living, long term care, and/or disability rights strongly preferred.
  • Knowledge of state and local housing policies, practices, laws and issues impacting seniors and people with disabilities preferred.
  • Willingness and ability to readily respond to changing circumstances and expectations.
  • Personal experience with disability a plus.

Independent Living Specialist (Rapid Transition Housing Program)

About the Position:

The Independent Living (IL) Specialist will report to the Executive Director and will work collaboratively with the Rapid Transition Housing Program (RHTP) Housing Specialist, as well as other programs at LICIL. Specific Duties:

  • Responsible for the establishment and delivery of Individualized Service Plans (ISP) with a focus on person-centered planning to meet the needs of RTH program participants to live independently, including but not limited to linkages to community resources, budgeting, transportation, and health care
  • Conduct monthly home visits for assessment, implementation, and revisions of
  • Work locally to establish mutual referral systems with Managed Care Organizations, Performing Provider Systems, hospitals, nursing homes, service providers,
  • Collaborate with applicants and/or their Housing Specialist, Transition Specialist, Nursing Home Discharge Planner, Care Manager (CM), natural support, or advocate to ensure a support services plan is in place for program admission and transition to independent living.
  • Track and report on all activities using the statewide project
  • Perform other duties and attend training as requested by the Program Director or Executive Director.

Qualifications:

  • Experience with the concepts of Independent Living, consumer control, and a strong commitment to promoting and advocating for the full integration of individuals with disabilities.
  • Experience transitioning individuals from institutions to the community including familiarity with finding affordable, accessible, integrated housing and the development of individualized service
  • Knowledge of state and local housing policies, practices, laws, and issues impacting seniors and people with disabilities strongly
  • Knowledge of the State’s Medicaid program and community resources which can assist people with disabilities in living
  • Willingness and ability to readily respond to changing circumstances and
  • Personal experience with disability a plus
